`#A`/`#B`/etc., but this wiki's renderer (comrak, `header_id_prefix = "h-"`)

match the real generated IDs. Also removed 187 empty `- **Term**` stub bullets
(bare CSV rows with no definition — zero information, pure duplication of blank
